Proof of Immunization Compliance

Dear New UNO Student:

As a new student, you are subject to a State legislated pre-matriculation immunization statue which requires that you provide UNO with your immunization status regarding MEASLES, MUMPS, RUBELLA, DIPTHERIA and TETANUS. In addition, a MENINGOCOCCAL immunization is required.  The document necessary for you to provide this status to the university’s Student Health Services can be viewed and downloaded by clicking on the following link:

Please understand that registration of your class schedule will not be allowed unless the completed form has been provided to the University.
